Suicide report: Zygier used a sheet to hang himself in jail

Ben Zygier hanged himself by tying a wet sheet to the bars of his prison window and using the other end as a noose, according to an internal investigation published Tuesday.

"Blame can be placed on a number of sources involved... including the senior command," stated the report, which was authored by Judge Daphne Blatman-Kadrai. "At the end of the day, the State will reconsider its position on the matter of prosecution."
