The cooling system at the tsunami-damaged Fukushima nuclear plant in Japan failed for the second time in a month on Thursday, officials said.A system that is meant to substantially reduce radioactive substances in tainted water at the plant has been halted because of an employee's operational mistake, The Japan Times reported.
